This form is for those supervisors who are either PLUS Members OR hold a subscription for our Supervisor Directory and who wish to cancel their listing

This form is for those supervisors who are either PLUS Members OR hold a subscription for our Supervisor Directory and who wish to cancel their listing 

1. Plus Members: please just complete the request and submit and provide us with a few days to remove your listing. As a PLUS member you can go back on the list whenever you like - you will just need to complete the application again

2. Directory Subscription Members: If you are asking to remove your listing then you are also asking to cancel your subscription, we require a minimum of 2 weeks notice prior to your renewal date (monthly or annual) to cancel. If you cancel under that time frame your request may not get actioned before it renews. Please complete the information below

Please indicate below that once you are removed from the directory that you will no longer be able to respond to requests for supervision in the FB group or Plus Community and that if you do your comments/posts will be removed.

Please indicate that you understand that once your subscription is cancelled your listing will be removed and you forfeit the current price you joined at if it has gone up. You will be charged the current price if you rejoin.
